Menopause: Blood Sugar Sensitivity & Insulin Resistance

In South Africa, the intersection of menopause, metabolic syndrome, and impaired glucose regulation represents a major public health concern. Declining estrogen levels combined with distinct local cardiometabolic risk profiles significantly elevate rates of insulin resistance, prediabetes, and type 2 diabetes among middle-aged women.



1. Prevalence of Metabolic Syndrome & Insulin Resistance


Relevant SA clinical studies demonstrate a sharp increase in Metabolic Syndrome (MetS) - where insulin resistance is the primary underlying driver, as women transition through menopause:


• Perimenopausal Women: ~47% have Metabolic Syndrome.

• Postmenopausal Women: ~61.4% have Metabolic Syndrome.


Local related epidemiological studies, report that over 41% of females present with elevated fasting blood glucose, compared to ~27% of age-matched males. Reduced high-density lipoprotein (HDL), a key marker of impaired lipid and glucose metabolism, afflicts nearly 40% of middle-aged South African women.



2. Abdominal Adiposity & Waist Circumference


Central adiposity (visceral fat) directly impairs peripheral insulin sensitivity by releasing free fatty acids and inflammatory cytokines into circulation:


• Over 58% of middle-aged South African adults screened in regional health assessments exhibit waist circumferences exceeding high-risk cut-offs (>84 cm for women).

• SA has among the highest female obesity rates in Sub-Saharan Africa. When obesity is superimposed on the menopausal transition, chronic low-grade cellular inflammation accelerates the progression from tissue insulin resistance to overt type 2 diabetes.



3. Diabetes Burden in SA Women


• According to Statistics SA, type 2 diabetes is the #1 leading natural cause of death among women, accounting for a disproportionate share of female mortality.

• Studies assessing diabetes risk across ethnic groups highlight that women of Black African and South Asian descent face elevated baseline risks for developing type 2 diabetes following menopause.
